Your First Appointment

What should I bring?

Please arrive at our office at least 15 minutes early to complete your necessary paperwork which includes filling out past medical history. We can also email these forms to you when you schedule your appointment, please mention this option the person scheduling you initial appointment.

We require a valid photo ID, insurance card, and current address for our records. A typical first introductory appointment can last anywhere between 30 minutes to an hour.

We appreciate your cooperation and look forward to providing you with the best experience possible, especially if you have never been to a Chiropractor before.

What are your payment options?

Most insurance plans are accepted at our office. We accept auto accident, workers compensation, personal injury and most health insurance plans, including Medicare. Not sure if your plan covers care in our office? We’ll be glad to help if you provide us your information.

All active firefighters and police officers now receive Chiropractic Care with zero co-pays. Please talk with our office manager prior to visiting.

No Insurance

For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. If you have no insurance or do not have chiropractic benefits there is still a way for you to receive the care you need. Many patients pay directly for care, as they discover chiropractic to be extremely cost-effective and affordable.
