OTHER EVALUATION SERVICES

 Our office provides PennDOT driver’s license physicals and exams for any individuals looking to obtain or renew their driver's license.  This includes first time drivers, commerical drivers, and those with a specific medical condition that require documentation to ensure they are fit for driving. We offer a thorough physical examination that mets PennDOT's requirements, to ensure compliance with state regulations. 

Bring all forms you need completed to your scheduled appointment.  

Please bring ID with you (see list below).

Approved forms of ID include:  a valid state identification card, a valid drivers license, a valid passport, a valid green card that includes an issue date and an expiration date. 

Minors, students and those without ID from the above mentioned list:

An employment ID or school identification card can be used however it must have a picture on the card and you will need to provide either your social security card or birth certificate (only originals can be accepted, all copies must have the proper validation on it).

Our office provides support for those completing an application and/or renewal for handicap parking placards.  Securing a handicap parking placard can make day-to-day tasks easier for any individual with a disability or medical conidition that limits their mobility. 

At your appointment we will provide you with the required documents needed to easily apply or renew a handicap parking placard.

 Comprehensive Chiropractic & Rehab offers thorough sports physicals for high school and college athletes, ensuring they are healthy and ready to compete. During a sports physical, Dr Cheng examines the indiviudal, gathering information about their healthand medical history.  This also includes a screening for conditions that may put student at a higher risk of injury, illness or other health problems that can result from physical activity.

 Please bring all forms that are needed to complete the physical to your scheduled appointment.
