As a new patient, please bring the following with you to your first appointment:
1) A valid photo ID for verification purposes
2) Your health insurance card (required for all motor vehicle accident, workers comp and personal injury patients as a backup)
3) Claim information, including your claim number, the adjuster's name and number
4) Attorney's contact information (only if you have retained one)
5) Radiology reports if you had studies done within the last year, CD of the images if available
Be sure to bring the payment amount that will be required with you! Below are our practice's accepted payment methods:
VISA, MasterCard, Discover, American Express, cards linked to HSA or FSA accounts, checks, and cash

As a new client coming in for a consultation and treatment session, please read the following information prior to your appt:

3) Bring an acceptable form of payment for the remaining balance on your account (we are not able to access the payment form you used when paying the deposit required to book the appointment)
4) Please arrive at least 5 minutes prior to the appointment time.
5) If you need to cancel or reschedule your appt, 24 hours notice is required to avoid a $50 fee being applied to your account. You can cell or text our office at 215-346-2155.
